If you do not want the Student Health Insurance Plan, you must decline or opt-out of coverage by submitting a waiver. You may only opt-out of coverage during the following Waiver Periods:

留学生は全員、University of South Alabamaの学生健康保険プラン（SHIP）に自動的に加入します。保険料は学生のアカウントに請求されます。学生が別の健康保険に加入している場合は、免除申請書を提出することができます。

SHIP（学生健康保険）の適用を免除するには、学生は大学のSHIPと同等の保険に加入していることを証明する書類を提出する必要があります。なお、免除申請は保険会社によって審査され、保険が有効であることが確認されますのでご了承ください。 

• Policy provides Medical Benefits (Ex. Both emergency and non-emergency health care and mental health care benefits) of at least $100,000 per accident or illness.
• A deductible no greater than $500 per accident or illness.
• Coverage for repatriation of remains (a minimum of $25,000 toward such expenses or, if an amount is not specified, the policy must specify coverage of all reasonable and necessary expenses for repatriation.)
• Medical evacuation coverage is equal to or greater than $50,000.
• The claims administrator is based in the United States and has a US telephone number, address for submission of claims. *Students will be responsible for submitting their own claims.
