If you do not want the Student Health Insurance Plan, you must decline or opt-out of coverage by submitting a waiver. You may only opt-out of coverage during the following Waiver Periods:

Tất cả sinh viên quốc tế đều tự động được đăng ký vào Chương trình Bảo hiểm Y tế Sinh viên (SHIP) của University of South Alabama. Phí bảo hiểm sẽ được tính vào tài khoản của sinh viên. Nếu sinh viên có bảo hiểm y tế khác, có thể nộp đơn xin miễn trừ.

• Policy provides Medical Benefits (Ex. Both emergency and non-emergency health care and mental health care benefits) of at least $100,000 per accident or illness.
• A deductible no greater than $500 per accident or illness.
• Coverage for repatriation of remains (a minimum of $25,000 toward such expenses or, if an amount is not specified, the policy must specify coverage of all reasonable and necessary expenses for repatriation.)
• Medical evacuation coverage is equal to or greater than $50,000.
• The claims administrator is based in the United States and has a US telephone number, address for submission of claims. *Students will be responsible for submitting their own claims.
